To master the clinical manifestation and treatment summary（概述） Neonatal cold injury is a disorder, which is due to coldness（寒冷） , premature birth（早產(chǎn)） , infection（感染） and asphyxia（窒息）. The clinical manifestations are hypothermia（低溫） associated with lethargy（嗜睡）, edema（水腫） and sclerema（硬化） of the skin, and some degree of systemic organ injury. Etiology and Mechanism 1.? ?Intrinsic factors（內因） 1) 溫度調節中樞（thermotaxic center ） 發(fā)育不成熟 2) 散熱過(guò)多（ excessive heat loss ） 3) 產(chǎn)熱不足，以棕色脂肪（ brown fat ）的化 學(xué)產(chǎn)熱方式為主，缺乏寒顫等物理產(chǎn)熱方式 4) 飽和脂肪酸（ saturated fatty acid ）含量多， 熔點(diǎn)高。 2. Extrinsic factors（外因） 1) 寒冷（coldness） 2) 攝入不足（intake deficiency） 3) 早產(chǎn)（premature birth） 4) 疾?。╠iseases） ①感染 ( infection） ②窒息（ asphyxia ） Clinical Manifestation ?1、General manifestation（一般情況） ? decreased activity（少動(dòng)）, feeble cry（少 哭）, poor feeding（少吃）, low temperature（低體溫） 2、Skin feature（皮膚表現） 1）cold, bright red, edema and sclerema （硬化） 2）gradation（順序） calf （小腿）? thigh（大腿）? Buttock （臀部） ? Face（面部） ? upper limb （上肢） ? Total body（全身） 3）Grades of severity （分度） According to an area of sclerema mild(輕度）: 50% 3、多器官功能損傷 circulatory collapse（循環(huán)衰竭） acute renal failure（急性腎功能衰竭） pulmonary hemorrhage（肺出血） respiratory failure （呼吸衰竭） Pulmonary hemorrhage（肺出血） Treatment ⒈ rewarming（復溫） ① body temperature 〉30 ℃ 將患兒置于中性溫度（neutral temperature） 的溫箱中，于 6 ~ 12hr恢復體溫。 ② body temperature〈30 ℃ 將患兒置于比肛溫高1~2 ℃的溫箱中，待肛 溫恢復至35 ℃時(shí)，維持箱溫的溫度于中性 溫度。 Open radiant warmer bed incubator Treatment ⒉熱量和液體供給 　Calorie: 50kcal／kg.d， 逐漸增加至 100~2 00kcal／kg.d Liquid: １ml／kcal ⒊對癥治療： 有微循環(huán)障礙者糾酸、擴容、血管活性藥。 有肺出血者行機械通氣 及時(shí)處理腎功能障礙及DIC ⒋預防感染
